Job description

Employment Type: Full-Time / Part-Time
Schedule: Monday–Friday, Day Shift (8am-2pm Minimum, can be up to 8 hrs daily)

We are seeking a compassionate and dependable Registered Nurse (RN) to join our Adult Day Health Care (ADHC) team. The RN will provide nursing care and health monitoring to older adults and adults with disabilities while working closely with our interdisciplinary team.

Responsibilities

  • Provide nursing assessments and monitor participants' health conditions.
  • Administer medications and treatments as ordered.
  • Monitor vital signs and blood glucose as needed.
  • Assist with chronic disease management and health education.
  • Assess fall risks, wounds, and changes in condition.
  • Respond to medical emergencies and coordinate emergency services when necessary.
  • Participate in Individual Plan of Care development and interdisciplinary team meetings.
  • Communicate with physicians, caregivers, families, and healthcare professionals.
  • Maintain accurate and timely medical documentation.
  • Follow infection-control, medication, safety, HIPAA, and regulatory requirements.

Qualifications

  • Current, unrestricted California RN license.
  • Current BLS/CPR certification.
  • Graduate of an accredited nursing program.
  • ADHC/CBAS, geriatric, community health, or similar experience preferred.
  • Strong communication, assessment, organization, and teamwork skills.
  • Compassionate and professional approach to working with seniors and adults with disabilities.
