Job Summary

SUMMARY
The Geriatric Clinical Pharmacist works in the patient care areas. Competent in reviewing and evaluation of medication orders for appropriate pharmacotherapy in the following age groups of patients: Neonatal, Pediatric, Adult and Geriatric. Demonstrate the ability to communicate with nursing staff and physicians to resolve and prevent drug related problems.

TheGeriatricClinical Pharmacist is a licensed pharmacist who is responsible for applying knowledge, skill, competence and clinical expertise in ourinpatient geriatric (ACE-accredited)nursing unit. The clinical pharmacist will work in theinpatient unitasa direct patient care provider in addition tofacilitatingquality initiatives for monitoring ofinpatientgeriatric therapy. The roles of this clinical pharmacist are that of advanced practitioner, educator, consultant, leader and researcher.

TheGeriatricClinical Pharmacistapplies his/her knowledge of, and clinical training in, pharmacotherapy to directly influence and impact physician prescribing to improve the use of medications throughout the Health System and to enhance positive drug therapy outcomes. The Clinical Pharmacist, acting as an integral member of the healthcare team, assumes responsibility for the appropriate selection and monitoring of anticoagulation therapy through direct patient care activities as well as serving as a liaison between the pharmacy and other healthcare practitioners.

Other functions include precepting pharmacy PGY-1 and PGY-2 residents anddoctoral-level pharmacy students, participating in staff development, creating opportunities for pharmacists to practice more clinically and promote rational and cost-effective use of medications at Grady Health System.

    QUALIFICATIONS

    • Doctorate of Pharmacy degree plus 1 year post-Phar.D. experience
    • ASHP accredited Residency in pharmacy practice or Doctor of Pharmacy degree at least 5 years of post-Phar.D. experience in job-related pharmacy practice, or Bachelor of Science degree in Pharmacy at least 6 years experience in job-related pharmacy practice at GHS
    • Fully licensed as a Pharmacist in the State of Georgia or, eligible for full licensure within 90 days through reciprocity or exam
