Job description

GENERAL SUMMARY:
The Community Health Worker (CHW) for the WIN Network primarily co-facilitates Group Prenatal Care (GPC) sessions, ensuring effective coordination, logistics, and participant engagement. The role includes identifying patients with clinical risk factors and facilitating appropriate referrals and follow-up care. CHWs provide targeted health education and coaching on prenatal care and healthy behaviors, conduct Social Determinants of Health (SDOH) screenings to identify and address barriers to care, and connect patients to community resources to support service utilization. Additionally, the CHW supports care coordination by serving as a liaison between patients and the clinical care team, promoting engagement and adherence to care plans.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
  • Co-facilitates Group Prenatal Care (GPC) sessions by coordinating activities, managing logistics, scheduling guest speakers, and ensuring a structured, welcoming environment for participants.
  • Identifies patients with clinical risk factors (e.g., hypertension, diabetes, low birthweight) and facilitates timely referrals to appropriate clinical programs and support services.
  • Delivers patient education and health coaching on prenatal care, nutrition, chronic disease management, and evidence-based healthy lifestyle practices.
  • Conducts Social Determinants of Health (SDOH) screenings to identify barriers to care, including housing, food insecurity, and transportation, and supports integration into care plans.
  • Coordinates connections to community-based resources, ensuring patients receive appropriate referrals and follow-up to promote service utilization.
  • Serves as a liaison between patients and the clinical care team, supporting care coordination, reinforcing treatment plans, and promoting patient engagement and adherence.

EDUCATION/EXPERIENCE REQUIRED:
• High school diploma or G.E.D. equivalent.
• Previous experience as a community health worker, preferred.
• Previous experience conducting health education, or working with underserved populations, preferred.
• Previous experience conducting home and/or clinic visits with patients and their primary care providers, preferred.
CERTIFICATIONS/LICENSURES REQUIRED:
Certification as a Community Health Worker or a willingness to become certified within six months of hire.
SKILLS & COMPETENCIES:
• Ability to build trust with the community and maintain ethical standards of care.
• Knowledge of local health resources and the social service system.
• Strong communication and interpersonal skills with the ability to work with diverse communities.
• Cultural competency, particularly with underserved or marginalized populations.
Must meet or exceed core customer service responsibilities, standards and behaviors as outlined in the Henry Ford Health Customer Service Policy
Must practice the customer skills as provided through on-going training and in-services.
PHYSICAL DEMANDS/WORKING CONDITIONS:
Depending on the specific needs of assigned work, there may be variations of in person, hybrid or remote office-based work with frequent community outreach, including home visits and attendance at community events. Must have a valid State of Michigan driver's license and automobile insurance.
