Western Geriatrics and Neurology, part of Brigade Health, is seeking a Primary Care Physician (MD/DO) to provide post-acute and long-term care services in Skilled Nursing Facilities (SNFs).
This role is ideal for physicians who enjoy caring for medically complex older adults, building strong relationships with facility teams, and practicing in a collaborative, value-based care model.

What You'll Do
• Provide primary and post-acute care in Skilled Nursing Facilities
• Manage admissions, follow-ups, and chronic care for geriatric patients
• Collaborate with nursing staff and interdisciplinary care teams
• Participate in care planning and quality initiatives
• Maintain timely documentation and patient communication
Qualifications
• MD or DO
• Board Certified / Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
• Active state medical license (or ability to obtain)
Experience in SNF, post-acute care, geriatrics, or long-term care preferred.
