Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Wellness Liaison to join our team in the Poughkeepsie.

Wellness Liaison is responsible for providing outreach and education services to coordinate intake and enrollment into Health Home and all Sun River Health service lines and programmatic services. The position requires strong interpersonal skills to make connections and build relationships with the target patient/client base. The Wellness Liaison works collaboratively with other departments as well as community-based agencies with the purpose of making linkages to health services for community members. The Wellness Liaison will work externally at Community Partner sites, health fairs and events or may be based in the SRH centers. The Wellness Liaison is an Ambassador of SRH whose activities reflect the mission and value of the organization and will provide outstanding customer service.

Essential Duties and Responsibilities for a Wellness Liaison:

  • Participates in outreach and educational events
  • Serves as liaison between the clients, providers, and account management staff
  • Provides outbound reminder calls to clients prior to their scheduled appointments.
  • Assists clients in securing appointments for SRH Health Center services.
  • Conducts Health Home pre-eligibility screenings and intakes for eligible clients.
  • Achieves intake monthly targets that are set on annual basis and adjusted quarterly as needed.
  • Aide with client outreach of clients, education of SRH services to community with a focus on linkage to Health Center and Health Home.
  • Assists in arranging transportation when needed.
  • Assists with health insurance application when needed.
  • Responsible for the complete, accurate and timely entry of data into required databases (eCW, TREAT, Salesforce-CRM), including but not limited to demographic information, insurance information, assessments, and progress notes.
  • Provide necessary documentation and information to Departmental Health Home Manager in each county/borough
  • Participates and attends Community Events such as health fairs; conducts on-street marketing of flyers and service information pamphlets
  • Provide support of Health Center and Health Home by participating in Kiosk/Mobile events in specified locations on schedule to engage community and conduct Health Home Intakes for health fairs, special events and locations.
  • Acts as initial service ambassador on the promotion and education to referral sources, clients, and community members of all SRH services while providing the highest quality service.
  • Supports SRH's mission, values, strategic goals and high standards of customer service.
  • Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information.
  • Participates in team meetings.

Minimum Education Requirement: High School Diploma/GED required with an additional four years of relevant experience in field of customer service in appropriate service industries. Bachelor Preferred.

Minimum Work-Related Experience: 2-4 years
