Position Overview

San Ysidro Wellness Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in Elizabeth, New Jersey. This full-time position offers the flexibility of remote work while providing essential mental health services to clients within a hospice center setting. You will play a vital role in assessing, diagnosing, and treating individuals facing major mental health challenges and severe emotional disorders, contributing significantly to their recovery and overall well-being.

Work Environment

This position operates primarily in a remote setting, allowing flexibility, and is closely integrated with our hospice center operations. You will collaborate with a multidisciplinary team that includes physicians, nurses, and other healthcare professionals to deliver comprehensive care. This role demands a strong focus on teamwork and communication as you work to serve individuals facing significant health challenges.

Key Responsibilities
  • Provide clinical assessments for assigned clients, discussing their presenting problems and developing personalized treatment plans that address their unique needs.
  • Deliver individual, group, and family therapy sessions, ensuring that each intervention is informed by evidence-based practices.
  • Facilitate effective communication and collaboration with clients, family members, and healthcare providers to coordinate care and enhance service delivery.
  • Conduct ongoing evaluations of clients' progress and adjust treatment plans as necessary to ensure successful outcomes.
  • Participate in crisis intervention efforts, providing timely responses to emergency situations affecting clients.
  • Maintain proper documentation and complete assessments within set timelines, ensuring compliance with organizational standards and ethical guidelines.
  • Carry a caseload of clients while ensuring their needs are met in a manner that prioritizes safety and empowerment.
  • Participate in quality improvement initiatives aimed at enhancing agency services and client satisfaction.
  • Assist in ensuring that safety protocols are followed, identifying and reporting any unsafe situations that may arise.
  • Provide on-call support during and after program hours as assigned, ensuring a continuous stream of care for clients.
Patient Population / Client Focus

Your work will primarily focus on clients experiencing major mental illnesses and severe emotional disorders. Many clients will come from diverse backgrounds, requiring a deep understanding of age-related issues and the unique needs of various populations. Your ability to connect with clients facing emotional distress in a compassionate manner will be crucial in facilitating their recovery and improving their overall quality of life.

Qualifications
  • A Master’s degree in Social Work (MSW) from an accredited program is required.
  • Current licensure as a Clinical Social Worker (LCSW) in New Jersey.
  • Expert knowledge in the assessment, diagnosis, and treatment of mental health conditions, as well as in crisis management.
  • Strong verbal and written communication skills, allowing effective interaction with clients and team members.
  • Demonstrated organizational and planning skills; must be able to manage a diverse caseload effectively.
  • Competency in addressing age-related and specialty issues relevant to our diverse clientele.
Preferred Experience
  • A minimum of 2 years of clinical experience providing direct services to individuals with mental health challenges.
  • Familiarity with hospice care and an understanding of the unique needs of clients in palliative care settings.
  • Proficient with electronic health record (EHR) systems, ensuring accurate documentation and timely updates.

San Ysidro Wellness Center offers a competitive salary ranging between USD 70,000 and 85,000 annually, based on experience and qualifications.
