As a Clinical Social Worker, you will work closely with a multidisciplinary team of healthcare professionals dedicated to improving patient outcomes and ensuring comprehensive care delivery.

• Provide direct clinical services to a designated caseload of clients, including assessment, individual, group, and family therapy.
• Conduct brief, evidence-based screenings and arrange for detailed assessments as required.
• Develop and implement integrated care plans that ensure access to necessary services and promote continuous information exchange among clients, their families, and care providers.
• Assist in crisis intervention efforts, employing appropriate approaches to ensure client safety and stability.
• Monitor client progress, ensuring timely adherence to treatment goals and documenting progress through comprehensive reporting.
• Actively participate in team meetings and committee work focused on quality improvement and service delivery enhancement.
• Ensure compliance with ethical guidelines and organizational standards to maintain integrity within clinical practices.
• Provide on-call coverage during designated hours to support clients in crisis situations or urgent needs.

The Clinical Social Worker will primarily serve a diverse population of clients experiencing major mental health challenges and emotional disorders within the reading community and surrounding areas. This role emphasizes culturally competent care, recognizing and respecting the varied backgrounds and personal narratives of each client. By adopting a client-centered approach, the LCSW will work tirelessly to empower individuals and foster positive life changes.

• A Master’s Degree in Social Work from an accredited institution.
• Active LCSW licensure in Pennsylvania.
• Strong understanding of mental health assessment, diagnosis, and treatment methodologies.
• Exceptional verbal and written communication skills to effectively engage with clients, families, and interdisciplinary teams.
• Proficient organizational and planning abilities to manage a caseload efficiently and ensure timely documentation.
• Competency in addressing age-related needs and developmental issues relevant to the client population.

A minimum of 2 years of clinical experience is preferred, particularly in community health settings or environments serving low-income populations. Experience with crisis intervention and collaborative care models is highly valued, as is familiarity with electronic health record systems that facilitate comprehensive client care.

This full-time position offers a competitive salary ranging from USD 70,000 to 85,000 per year, commensurate with experience and qualifications.
