Registered Nurse (RN) - Home Health
Location: Floyd county, Riner, Pilot, and Christiansburg, Virginia
Position Type: Full-Time, Salaried

Warm Hearth at Home is seeking a compassionate and motivated Registered Nurse (RN) to provide skilled home health services throughout Floyd County, Riner, Pilot, and Christiansburg. Our team is dedicated to helping patients remain safely and comfortably in their homes while receiving high-quality care.
Position Summary
The Home Health RN is responsible for conducting patient assessments, developing and managing individualized care plans, providing skilled nursing services, coordinating care with physicians and interdisciplinary team members, and educating patients and caregivers.
Responsibilities
  • Complete Start of Care (SOC), Recertification, Resumption of Care (ROC), and Discharge assessments
  • Provide skilled nursing visits in patients' homes
  • Develop and update individualized plans of care
  • Monitor patient progress and communicate changes to physicians
  • Educate patients and caregivers on disease management, medications, and safety
  • Coordinate care with therapy, social work, hospice, and other healthcare professionals
  • Maintain accurate and timely clinical documentation
  • Participate in on-call rotation as assigned

Qualifications
  • Current Virginia Registered Nurse (RN) license
  • Minimum one year of nursing experience preferred
  • Home health experience preferred but not required
  • Strong assessment, communication, and organizational skills
  • Valid driver's license and reliable transportation
  • Ability to work independently while collaborating with a supportive team
