We are seeking a dedicated and compassionate full-time Registered Nurse Case Manager to join our healthcare team. In this rewarding role, you will coordinate and oversee the individualized care of patients, ensuring they receive the highest standard of medical services in their home environment. This position offers the opportunity to make a meaningful impact in the lives of patients and their families while working within a supportive interdisciplinary team.


Schedule: Full-Time, Weekdays, On-Call (not required, but appreciated), Day Shift


Key Responsibilities:

  • Coordinate and manage the comprehensive care of assigned patients.
  • Develop, implement, and regularly update individualized care plans.
  • Perform skilled nursing assessments and provide direct patient care.
  • Collaborate with physicians, therapists, social workers, and other healthcare professionals.
  • Educate patients and families on disease processes, medications, treatments, and care plans.
  • Ensure timely and accurate documentation of all care activities.
  • Participate in case conferences and interdisciplinary team meetings.
  • Serve as a compassionate resource for patients and families, providing emotional support and guidance.

Requirements

  • Active Registered Nurse (RN) license in the state of Pennsylvania.
  • Previous experience in home health, hospice, or case management preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication, organizational, and time-management abilities.
  • Compassionate, patient-centered approach.
  • Valid driver's license, reliable transportation, and current auto insurance.

Physical Requirements:

  • Ability to lift and transfer patients, as well as carry equipment and supplies weighing up to 50 pounds.
  • Capable of prolonged standing, bending, squatting, and walking during home visits.
  • Fine motor skills for documentation and administering treatments.
  • Ability to respond swiftly and efficiently to emergency situations.
  • Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces.
  • Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
  • Job requires accepting criticism and dealing calmly and effectively with high-stress situations.
  • The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
