Job description

We are seeking an experienced Clinical Manager (RN) Full-Time to lead our home health team. This role oversees clinical operations, ensures compliance with Medicare Conditions of Participation, and supports staff in delivering high-quality, patient-centered care in the home setting.

Responsibilities

Supervise and support nurses, therapists, and aides

Ensure care is delivered per physician orders and individualized care plans

Review documentation for accuracy, timeliness, and compliance (OASIS experience preferred)

Maintain compliance with Medicare, Medicaid, and state regulations

Lead case conferences and collaborate with the interdisciplinary team

Participate in audits, QA/PI initiatives, and survey readiness

Assist with hiring, onboarding, and staff development

Monitor productivity, scheduling, and clinical performance

Address patient and staff concerns promptly


Qualifications

Active RN license in Utah

Minimum 1-2 years home health experience required

1+ year leadership/supervisory experience preferred

Strong knowledge of Medicare CoPs and home health regulations

OASIS and EMR experience preferred

Excellent leadership, organization, and communication skills
