As a Home Health & Hospice RN, you will:

  • Provide skilled nursing care to patients in their homes, primarily in Plymouth and surrounding areas.
  • Utilize your critical thinking and assessment skills to develop and implement individualized care plans.
  • Perform a range of essential nursing skills, including wound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage.
  • Provide LNA supervision.
  • Work independently while being a valued member of our collaborative team.
  • Maintain accurate and timely documentation.
  • Participate in a rotating weekend and on-call schedule to ensure continuous patient support.
  • Travel to patient homes in various weather conditions.

What You Need to Succeed:

  • Required: Current New Hampshire Registered Nurse (RN) license.
  • Required: Valid Driver's License and insured vehicle.
  • Essential: Minimum of one year of professional nursing experience.
  • Key Skills: Ability to work independently, strong self-motivation, excellent organizational skills, and the ability to travel.
  • Clinical Expertise: Demonstrated proficiency in wound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage.
  • Education: Associate's degree or higher in nursing.

Work Environment:

  • On the road: Traveling to patient homes within the designated service area.
  • Day shift with a rotating weekend and on-call supplemental schedule.
