Key Responsibilities
  • Conduct thorough psychosocial assessments of individuals, families, and groups to understand their specific challenges and strengths.
  • Develop, implement, and adjust individualized treatment plans in collaboration with patients, employing evidence-based methods such as Cognitive Behavioral Therapy (CBT) and other therapeutic interventions.
  • Facilitate regular case management meetings with interdisciplinary teams to coordinate comprehensive care, ensuring all aspects of the patientโ€™s needs are holistically addressed.
  • Document all patient interactions and treatment plans in accordance with ethical practices and organizational standards, ensuring timely updates for accurate care continuity.
  • Provide ongoing support and counseling services to assist patients and families in achieving their established therapeutic goals.
Patient Population / Client Focus

Your role as a Mental Health Counselor will primarily focus on engaging with clients in a hospice setting, where emotional support and mental resilience are critical. You will work with a diverse range of patients, including individuals facing life-limiting illnesses and their families, providing compassionate care tailored to the unique emotional landscapes experienced during such transformative journeys.

Qualifications
  • A valid state license as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or a Doctor of Psychology (PsyD).
  • A minimum of two years of clinical experience in mental health settings, showcasing your ability to handle complex cases effectively.
Preferred Experience

While we require at least two years of experience, candidates with specific background in hospice care or palliative counseling are especially encouraged to apply. Additionally, proficiency in using electronic health record (EHR) systems will be considered a plus, enhancing your efficiency in documentation and patient tracking.

Compensation and Benefits

We offer a competitive salary range from USD 60,000 to 104,000 yearly, commensurate with your background and qualifications.
