• Conduct comprehensive mental health assessments that encompass clients' medical history and current health conditions.
• Develop and execute personalized treatment plans that address both psychological and medical issues, ensuring a holistic approach to care.
• Provide individualized and group therapy sessions based on clinical best practices, guiding clients through various therapeutic modalities such as cognitive-behavioral therapy (CBT) and solution-focused therapy.
• Foster collaboration with healthcare professionals to ensure cohesive treatment strategies and improved patient outcomes.
• Implement crisis intervention strategies during emergencies, offering immediate support and resources.
• Educate clients about the relationship between mental and physical health, equipping them with knowledge and strategies to manage their health more effectively.
• Maintain thorough and confidential documentation of patient interactions in accordance with legal and ethical standards.
• Participate actively in case conferences, team meetings, and interdisciplinary care planning to enhance treatment effectiveness.
• Stay informed on current research and advancements in integrated care approaches to continually improve service delivery.

In this role, you will work primarily with individuals coping with co-occurring medical and mental health conditions, including autoimmune disorders, substance use disorders, and chronic pain syndromes. Your specialized skills will provide essential support to clients navigating these complex health challenges, enhancing their overall well-being and quality of life.

• A Bachelor's degree in Social Work, Mental Health Counseling, Psychology, or a related field.
• Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or Licensed Marriage and Family Therapist (LMFT) in the state of New York.
• Strong interpersonal and communication skills, enabling effective client engagement.
• Ability to uphold professional boundaries and maintain confidentiality in patient interactions.
• Proficient in the use of electronic health record systems and other relevant software to document client information accurately.
• Capability to work collaboratively as part of an interdisciplinary team, contributing to collective patient care goals.

• Previous experience in a community health setting or working with clients who have co-occurring medical and psychological conditions.
• Familiarity with integrated care practices and collaborative treatment models.
• Experience utilizing telehealth platforms for service delivery.

The salary for this full-time position ranges from $90,000 to $100,000 annually, commensurate with experience and caseload. We provide a competitive benefits package that includes:
