• Conduct thorough psychosocial assessments of patients through interviews, observations, and meticulous reviews of medical records to identify emotional, social, and environmental factors affecting their recovery process.
• Deliver compassionate counseling and support to patients and their families, addressing adjustment issues, emotional well-being, and coping strategies to facilitate their rehabilitation journey.
• Collaborate with a multidisciplinary rehabilitation team to develop personalized treatment plans that address the psychosocial needs of each patient.
• Assist patients in identifying and accessing community resources, such as support networks, vocational rehabilitation programs, or housing assistance, to aid in their holistic recovery.
• Provide education and support to patients' families regarding available community resources and services following rehabilitation.
• Actively participate in discharge planning to ensure smooth transitions for patients returning home or moving to other care settings, emphasizing continuity of care.

The LCSW will primarily work with individuals facing significant challenges related to health conditions, disabilities, or life-altering events. • Master's degree in Social Work from an accredited institution.
