Job Summary: The Patient Navigator functions as the coordinator and clinical resource to support patients throughout their care journey and promote continuity of care across the practice.  The Patient Navigator will serve as a resource and liaison for patient referrals, communication, and patient triage.

Minimum Requirements:
   Registered Nurse graduate of approved School of Nursing. Associate's degree required. Bachelor's degree preferred.
   Minimum of 2 nursing years' experience with cardiology or electrophysiology preferred.
   Ability to review systems and schedules to identify gaps and need for improvements.
   Develop relationships and excellent communication skills to coordinate care with multiple departments.
   Ability to show initiative and provide proactive solutions to complicated systems with large volumes of patients
