Job Details
The Nurse Navigator performs basic nursing skills, patient navigation, and data management in the outpatient setting, with a focus on enhancing both the patient experience and the quality of interdisciplinary medical care provided by this Korman Respiratory Institute Program of Excellence. The Nurse Navigator will assist patients undergoing lung cancer screening through the comprehensive, centralized Lung Cancer Screening Program at Jefferson Health. A critical quality for success in this position is the ability to develop high-functioning interpersonal relationships with patients, physicians, and scheduling services for navigation of the Jefferson Health system.Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
Assists with scheduling, shared decision-making, and education for patients undergoing lung cancer screening, supervised by a CRNP.
Opportunity to obtain master tobacco treatment specialist (MTTS) certification. Training will be provided.
Reviews test results and coordinates additional workup for patients, including follow-up scans, diagnostic procedures, and/or evaluation by additional subspecialists in Pulmonary Medicine or in the NCI-designated Sidney Kimmel Cancer CenterRecords results and tracks patients who are due for follow-up in the Lung Cancer Screening Program using state-of-the-art radiology tracking software. Training will be provided.
Leads community outreach efforts together with support organizations to provide education and increase enrollment in lung cancer screening.
Participation and collaboration on clinical research projects focused on early detection of lung cancer will also be expected as a minor part of this role.
