The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE:
Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.

Full time 40 hours M-F 8-4:30; no weekends or holidays

The NN specializes in teaching pulmonary disease and bronchoscopy procedures and will provide comprehensive medical navigation for patients throughout the pulmonary diagnostic and treatment continuum. will support patients from initial pulmonary referral and lung cancer screening through diagnostic bronchoscopy, diagnosis disclosure, and transition to treatment or ongoing surveillance. The navigator serves as a central point of contact, ensuring timely, coordinated, patient-centered care while collaborating closely with the Nurse Manager, Assistant Nurse Manager, and pulmonary providers, Identifies resources for patient self-management skills, Identifies barriers to a successful care management path and develops strategies to overcome them, Participates in the development of evidence-based protocols, policies, workflows, guidelines, and flow sheets within the medical home model, Keeps abreast of organizational developments and practices that may impact operations by engaging in continuing education, professional organizations, seminars, and relevant literature. Registered Nurse, BSN/ Master's preferred.
