Calibre Ortho Care is seeking dedicated and compassionate Orthopedic Physical Therapists to join our full-time team in Elgin, Illinois. This onsite position focuses on enhancing the lives of patients of all ages through effective physical therapy interventions tailored to their individual needs. This role is based in a home care setting, allowing you the flexibility to work onsite with patients in their own environments. • Conduct thorough evaluations of patients’ physical conditions, implementing evidence-based assessments to identify specific needs and challenges.
• Develop customized treatment plans aimed at achieving optimal recovery, with measurable objectives tailored to each patient’s unique goals.
• Utilize advanced tools and methodologies to regularly reassess progress, making necessary adjustments to treatment plans to ensure responsiveness to patient needs.
• Document all interactions and treatment outcomes accurately using our electronic record system, ensuring compliance with regulatory standards.
• Educate patients and their families on the rehabilitation process, providing guidance on exercises, pain management strategies, and the prevention of future injuries.
• Collaborate with a multidisciplinary team, sharing patient insights and strategies that optimize synergistic care for clients receiving multiple therapy services.
• Actively promote Calibre Ortho Care’s values and mission, serving as an ambassador for the quality of care we deliver.

You will work with a diverse range of clients, including children, adults, and seniors, each presenting unique orthopedic challenges. Our patients come from various backgrounds and cultures, requiring a therapist who is adaptable and sensitive to the needs of individuals from all walks of life.

• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• A current and unrestricted license to practice physical therapy in the state of Illinois.
• Proficiency in both in-person evaluations and treatments as well as telehealth services when appropriate.
• A genuine passion for working with a diverse patient demographic, demonstrating empathy and understanding.
• Strong ability to adapt to changing situations in a dynamic healthcare environment.
• Excellent collaborative skills, with a focus on team-oriented patient care and exceptional customer service.

• Prior experience in an orthopedic-focused physical therapy role is highly desirable.
• Experience with pediatric and geriatric populations is a plus.
• Familiarity with electronic health record (EHR) systems and documentation practices.
• A background in multidisciplinary teamwork and integrated care approaches.

The compensation for this role ranges from USD 90,000 to 110,000 annually, commensurate with experience and education. In addition to a competitive salary, we offer a comprehensive benefits package that includes:
