Speech-Language Pathologist (SLP) – Full Time & Per Diem

Nancy Hart Nursing Center

Location: 2117 Dr. George Ward Road, Elberton, GA 30635

Nancy Hart Nursing Center is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team in a skilled nursing and subacute rehabilitation setting. We are currently accepting applications for Full-Time and Per Diem opportunities.

The Speech-Language Pathologist will provide skilled speech, language, cognitive, and swallowing therapy services to residents, helping them improve communication, enhance safety, and maximize their functional independence and quality of life.

Responsibilities:

  • Evaluate residents and develop individualized speech-language therapy treatment plans
  • Provide skilled therapy services addressing speech, language, cognitive, communication, and swallowing needs
  • Perform swallowing evaluations and provide appropriate treatment interventions
  • Monitor resident progress and collaborate with the interdisciplinary care team
  • Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries
  • Educate residents, families, and caregivers on therapy goals, strategies, and recommendations
  • Collaborate with nursing staff, physicians, and other rehabilitation professionals to support comprehensive resident care

Qualifications:

  • Current Speech-Language Pathologist license in the state of Georgia
  • Master’s degree from an accredited Speech-Language Pathology program
  • Certificate of Clinical Competence (CCC-SLP) preferred
  • Experience in skilled nursing, long-term care, subacute rehabilitation, or similar healthcare settings preferred
  • Strong clinical, communication, and organizational skills
  • Commitment to providing compassionate, resident-centered care
