Job description

Lakeland Nursing & Rehabilitation in Haskell, NJ is hiring a full-time Nurse Practitioner (NP). Lakeland is a 201-bed skilled nursing and rehabilitation center in Passaic County that has been serving the Haskell community for years, known for a warm, homelike atmosphere and a rehab program that runs seven days a week with a state-of-the-art therapy gym. As our Nurse Practitioner, you'll work alongside a team of in-house, board-certified specialists managing everything from short-term rehab patients to long-term care residents. This is a great fit for a Nurse Practitioner who wants variety in their caseload and a supportive, established clinical team around them.

We're looking for a Nurse Practitioner (NP) with at least 6 months of NP experience. Nursing home experience is preferred โ€” an NP who has worked in a skilled nursing or long-term care setting will hit the ground running here.

What You'll Do

  • Provide direct patient care and medical management across short-term rehab and long-term care residents

  • Perform assessments, manage chronic and acute conditions, and update treatment plans in collaboration with attending physicians

  • Round on residents and respond to changes in condition

  • Communicate with residents, families, and nursing staff to keep care coordinated

Qualifications

  • Active NJ Nurse Practitioner (NP) / Advanced Practice Nurse (APN) license

  • Minimum 6 months of experience as a Nurse Practitioner (NP)

  • Experience in a skilled nursing facility (SNF) / long-term care (LTC) setting preferred
