Position Overview

Productive Mind Counseling is seeking dedicated and compassionate Licensed Mental Health Counselors to join our remote team at a Hospice Center in Uniondale, New York. This full-time position offers a unique opportunity to provide critical mental health support to patients facing life-altering illnesses, while working collaboratively with a diverse team of professionals. Work Environment

This role is primarily remote, allowing for flexibility and adaptability in your working hours, while still providing essential support to hospice patients. While the majority of therapy sessions will occur via telehealth platforms, you will be part of a collaborative network, regularly engaging with a multidisciplinary team of medical professionals, support staff, and other therapists. Regular team meetings and training sessions will ensure that you stay connected with your colleagues and are continuously updated on best practices and advancements in mental health care.

Key Responsibilities
  • Conduct thorough assessments and screenings to evaluate the mental health needs of hospice patients, developing individualized treatment plans tailored to their unique challenges.
  • Provide short-term individual counseling and crisis intervention services, helping patients navigate emotional distress related to their diagnoses.
  • Facilitate therapy groups and workshops that address topics such as grief, coping strategies, and emotional well-being, fostering a sense of community among participants.
  • Collaborate with an interdisciplinary team including physicians, nurses, and social workers to ensure comprehensive patient-centered care.
  • Engage in outreach initiatives, such as mental health awareness campaigns and educational programs aimed at improving community understanding of hospice and mental health.
  • Maintain accurate and confidential patient records in compliance with HIPAA regulations and ethical standards.
Patient Population / Client Focus

Your work will focus on a diverse patient population within the hospice setting, where patients often deal with not only physical pain but also complex emotional and psychological issues. Many individuals may experience intense feelings of grief, anxiety, or depression related to their prognosis, making your role essential in providing support and tools for emotional resilience during challenging times.

Qualifications
  • Must possess an active state license as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).
  • A Master’s or doctoral degree in Counseling, Psychology, Social Work, or a related field is required.
  • Demonstrated understanding of the unique mental health challenges faced by patients within hospice care.
  • Exceptional communication and interpersonal skills, capable of engaging empathetically with patients and families from varied backgrounds.
  • Expertise in evidence-based therapeutic approaches and brief therapy models applicable to a hospice counseling context.
  • Ability to manage a diverse caseload, thriving in a fast-paced environment while ensuring high-quality, compassionate care.
Preferred Experience
  • Previous experience working in hospice care or palliative settings is highly desirable.
  • Background in trauma-informed care and familiarity with end-of-life issues will be advantageous.
  • Prior experience in telehealth counseling is preferred, demonstrating adaptability to remote therapeutic practices.
  • Proven history of successful collaboration within a multidisciplinary team.
