Compensation: $75,000+ย / FFSย DOE

Status: Full-Time, W2 Employee

Schedule: Monday - Friday, 8 AM - 5 PM

Position Overview

We are seeking an experienced and compassionate Mobile Licensed Behavioral Health Counselor to support a geriatric patient population within a mobile primary care practice. This role partners closely with medical/psychiatric providers to deliver integrated, patient-centered behavioral health services directly in patients' homes, assisted living facilities, and other community settings.

This position is ideal for clinicians who are highly independent, organized, and comfortable working with medically complex older adults. The counselor is responsible for managing their own schedule, providing in-person counseling services, and ensuring all documentation meets Medicare and regulatory compliance standards.

Providers are independent contractors. Compensation is fee-for-service, based on completed visits and submission of compliant documentation.

Required Licensure

  • Candidates must hold an active, unrestricted license in one of the following:
  • Licensed Professional Counselor (LPC)
  • Licensed Clinical Social Worker (LCSW)
  • Clinical Psychologist (PhD or PsyD)

Key Responsibilities

Geriatric Behavioral Health Care

  • Provide individual therapy and supportive counseling to geriatric patients (primarily Medicare population)
  • Address common conditions such as depression, anxiety, adjustment disorders, grief, cognitive decline, and chronic illness-related behavioral health concerns
  • Conduct comprehensive psychosocial and behavioral health assessments
  • Develop and implement individualized, goal-oriented treatment plans
  • Utilize evidence-based, age-appropriate therapeutic interventions

Integrated Care Collaboration

  • Work closely with mobile primary care and psychiatric providers to support whole-person care
  • Communicate clinical findings and collaborate on care plans for patients with complex medical and behavioral needs
  • Support caregivers and family members with education, coping strategies, and care planning

Mobile Service Delivery

  • Provide care in patients' homes, assisted living facilities, and group homes
  • Adapt clinical approach to non-traditional treatment environments
  • Maintain professionalism, safety awareness, and flexibility in the field

Scheduling & Productivity

  • Independently create, manage, and maintain a consistent weekly schedule
  • Coordinate directly with patients, caregivers, facilities, and referral sources
  • Optimize scheduling routes and volume to maximize productivity and continuity of care
  • Multiple providers will be referring for steady influx of patients

Documentation & Medicare Compliance

  • Complete timely, accurate, and defensible clinical documentation for every visit
  • Ensure documentation supports medical necessity in accordance with Medicare guidelines
  • Maintain compliance with:
  • CMS regulations
  • HIPAA requirements
  • Internal policies and quality standards
  • Submit documentation promptly to support billing and revenue cycle operations

Compensation

  • Fee-for-service model
  • Payment based on:
  • Completed patient visits
  • Timely submission of compliant documentation
  • No guaranteed salary; compensation is directly tied to productivity and efficiency

Qualifications

  • Active, unrestricted license (required)
  • Experience working with geriatric or medically complex populations strongly preferred
  • Knowledge of Medicare documentation standards and medical necessity criteria preferred
  • Experience in home-based, community-based, or integrated care settings a plus
  • Strong clinical, organizational, and independent decision-making skills
  • Reliable transportation and willingness to travel within assigned territory
  • Proficiency in EMR/EHR systems
