Work Environment

This role is primarily onsite, as you will deliver therapy services directly within patients' homes. You will collaborate with an interdisciplinary team in a dynamic environment, adapting to each unique home setting while ensuring a respectful and empathetic atmosphere. The position involves regular travel within Cypress, Florida, and its surrounding areas, where you will assess and modify treatment plans based on the patient's home environment and personal circumstances.

Key Responsibilities
  • Perform comprehensive assessments of patients, utilizing diagnostic and prognostic evaluations to understand their physical capabilities and devise appropriate treatment plans.
  • Administer therapeutic interventions, including manual therapy, functional exercises, and treatments using physical agents, to address specific patient needs.
  • Supervise and mentor Physical Therapy Assistants, ensuring they understand and execute the prescribed treatment protocols while complying with state regulations.
  • Evaluate home environments critically before making treatment recommendations that foster independence and improve rehabilitation prospects.
  • Compile and maintain meticulous clinical records—such as initial evaluations, progress notes, and discharge summaries—to support patient care documentation.
  • Engage with patients and their families, providing education on physical therapy practices, the use of therapeutic devices, and necessary care to ensure compliance and continuity of care.
  • Collaborate effectively with case managers and other healthcare professionals in interdisciplinary meetings to discuss treatment plans and progress updates.
  • Document and submit clinical information and payroll reports timely in accordance with organizational policies.
  • Continuously assess and modify treatment plans based on patient feedback and observed outcomes, keeping the physician informed of significant changes in patient conditions.
Patient Population / Client Focus

In this role, you will serve a diverse population, focusing on individuals recovering from surgeries, injuries, and chronic conditions that affect mobility and daily functionality. Many of our patients are older adults or those with disabilities who require specialized, compassionate care to regain their independence. Your therapeutic interventions will have a profound impact on their recovery journey, ultimately aiming to restore optimal physical function and enhance their quality of life.

Qualifications
  • A Bachelor’s degree in Physical Therapy from an accredited institution is required; a Doctorate in Physical Therapy (DPT) is preferred.
  • Current and valid physical therapy license in the state of Florida.
  • Proven track record of at least two years of professional experience in physical therapy, preferably within a home care or acute care setting.
  • Current certification in CPR and a valid driver’s license with automobile insurance.
  • Demonstrated ability to communicate effectively with patients, families, and team members, ensuring a collaborative approach to care.
  • A strong commitment to clinical excellence and a passion for enhancing patient experiences through focused, high-quality therapy.
Preferred Experience

Experience in home healthcare and familiarity with electronic health record systems is highly desirable. Proficiency in developing individualized care plans that cater to the specific needs of patients will give candidates a distinct advantage.
