This full-time position allows for a remote work setting while primarily contributing to our outpatient office services. Key Responsibilities
  • Conduct comprehensive and ongoing assessments for patients to determine their specific needs and formulate personalized treatment plans.
  • Facilitate communication among supervisors, care staff, and family members to monitor and discuss each clientโ€™s progress effectively.
  • Develop and present tentative schedules for appointments, home visits, and progress observations to supervisors to ensure a structured approach to treatment.
  • Coordinate home visits, performing observations, chart reviews, and staff interviews to obtain a thorough understanding of client circumstances.
  • Ensure that treatment plans are implemented effectively and adjusted as needed based on client feedback and outcomes.
  • Participate in interdisciplinary team meetings to discuss patient care strategies and contribute to the continuous improvement of treatment protocols.
  • Document monthly and quarterly progress notes to track patient advancements and changes in their treatment journey.
  • Lead training sessions to ensure staff are competent in implementing treatment plans and understanding documentation requirements.
  • Engage in follow-up visits and ongoing evaluations to ensure consistent support for clients' evolving needs.
  • Maintain accurate and accessible records, assuring compliance with legal and ethical standards of practice.
  • Act as a liaison with the community and mental health resources to foster partnerships that support client objectives.
  • Collaborate on creating and refining training materials aimed at improving programs and services for patients.
