Position Overview

Advanced Cardiology PC is seeking a dedicated Full-Time Nurse Practitioner to join our dynamic healthcare team in Syracuse, New York. In this vital role, you will provide exceptional primary care to adults, assisting them in managing their health and improving their quality of life. This position is primarily onsite in a hospital setting, where you will work integrally with a multidisciplinary team.

Work Environment

You will be part of a collaborative care team that includes physicians, nurse practitioners, physician assistants, medical assistants, and support personnel, including committed registered nurses and care managers. This team-oriented environment is designed to provide a seamless experience for patients, allowing healthcare providers to focus on delivering high-quality primary care, care coordination, and patient education.

Key Responsibilities
  • Deliver outstanding primary care to patients through thorough assessments and patient-centered treatment plans.
  • Conduct office visits for both routine check-ups and acute health concerns, ensuring comprehensive health evaluations and prompt issue resolution.
  • Administer annual wellness visits and health risk assessments, placing emphasis on holistic health, accurate documentation, and personalized patient experiences.
  • Coordinate care with various specialists and healthcare agencies, ensuring that each patient receives tailored services and follow-ups according to their specific needs.
  • Lead population health initiatives, ensuring compliance with health screenings and preventative measures, such as guiding eligible patients through evidence-based breast cancer screenings every two years.
  • Assist the care team with phone triage and outreach, delivering support and guidance to patients regarding their health inquiries.
  • Educate patients on their health conditions, care plans, and treatment options, fostering an environment where patients feel empowered to take charge of their health.
  • Engage in promotional activities aimed at enhancing community presence and patient education initiatives.
  • Conduct home visits for patients who require it, ensuring continuity of care in varied settings.
  • Occasionally report to the Medical Director while closely liaising with operational leadership to improve clinic functions.
Patient Population / Client Focus

The primary focus of this role will be on older adults, particularly those enrolled in Medicare. By dedicating our services to this demographic, we prioritize their health and well-being, ensuring that our care model addresses the specific challenges and requirements that come with aging. Our overarching goal is to maintain their health, promote independence, and help them lead fulfilling lives.

Qualifications
  • A Master of Science in Nursing or a Doctor of Nursing Practice, or successful completion of an NP certificate program.
  • National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
  • Active, non-probationary state Nurse Practitioner license in New York.
  • Current DEA license, allowing you to prescribe medications as needed.
  • Proven experience in primary care settings, ideally including internal medicine, geriatrics, and family medicine.
  • Strong understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding requirements, and health risk assessment protocols.
  • A collaborative spirit and a genuine passion for delivering patient-centered care.
Preferred Experience
  • Experience using electronic health record (EHR) systems for documenting patient interactions and outcomes.
  • A background in community-based healthcare or innovative care models targeting older adults.
  • Previous involvement in population health initiatives and quality improvement processes.
Compensation and Benefits

The position offers a competitive salary range of USD 120,000 โ€“ 160,000 per year, commensurate with experience.
