Job description

Primary Care NP/PA โ€“ Make a Real Impact in Home and Community Medicine
Territory: Englewood, CO
Join the team thatโ€™s redefining primary care.
Bloom Healthcare is an award-winning, employee-owned medical practice delivering high-touch, high-quality care to medically complex patients where they areโ€”at home and in communities. Our innovative model consistently outperforms national benchmarks and has earned us โ€œTop Workplaceโ€ honors six years running. We invest in our people first, because exceptional clinicians create exceptional patient outcomes.
If youโ€™re a Nurse Practitioner or Physician Assistant who wants meaningful autonomy, manageable patient volumes, and a mission-driven culture that truly values youโ€”Bloom is the place to build your career.

Why This Role Matters
As a Primary Care Provider, youโ€™ll be a lifeline for homebound seniors, helping them age safely and comfortably in place. You will provide relationship-based primary care, respond to acute needs, and drive measurable improvements in health outcomesโ€”all with full support from Bloomโ€™s interdisciplinary team and advanced data systems.
This is the ideal role for a clinician who thrives on independence, enjoys complex primary care, and wants to practice medicine the way it was meant to be: patient-centered, unrushed, and deeply impactful.

What Youโ€™ll Do
  • Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities.
  • Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership.
  • Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
  • Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
  • Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
  • Utilize Bloomโ€™s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes.
  • Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.

Whatโ€™s in It for You
A manageable, quality-focused schedule
  • Monday - Friday
  • 8-12 patients per day (no unrealistic volume expectations)
  • Light call rotation: only 2โ€“3 times per year
  • Weekends off and flexible daily workflows
Robust benefits โ€“ starting Day 1
  • 100% employer-paid Health, Dental & Vision effective Day 1
  • 3 weeks PTO + 7 paid holidays
  • 401(k)
  • Employer-paid Basic Life & AD&D
  • CME allowance + paid licensing & malpractice
  • Strong support from MDs, triage, schedulers, IT, billing, and marketingโ€”so you can focus on patient care
Meaningful autonomy + strong support
You practice independently, but never alone. Our interconnected care teams and advanced analytics ensure you always have what you need to make excellent clinical decisions.

Youโ€™ll Thrive Here If You Areโ€ฆ
  • A NP or PA with a masterโ€™s degree and active Colorado licensure
  • Passionate about serving seniors and medically complex populations
  • Skilled at building trust, communicating clearly, and adapting to home-based environments
  • Motivated by mission, relationship-driven care, and meaningful outcomes
  • Ready to join a team that values your voice, expertise, and well-being

Compensation
$100,000 โ€“ $125,000 annually, depending on experience, with opportunity for performance-based bonuses and full benefits.
