Job Title: Hospice Associate Medical Director (MD/DO)
Location: Firestone, Colorado Area

Job Summary
The Associate Medical Director (AMD) provides clinical leadership and medical oversight for Bloom's Hospice Program, blending patient care, regulatory compliance, and interdisciplinary guidance to ensure compassionate, high-quality end-of-life care aligned with patient goals and Bloom's integrated model.
Key Responsibilities
Clinical Care & Medical Oversight
  • Serve as attending/supervising physician for hospice patients, managing symptom control, hospice evaluations, and care direction (primarily remote/telehealth).
  • Assist in the face-to-face encounter process (third benefit period+) on a limited, as-needed basis only.
  • Review, certify, and recertify terminal illness with compliant, clinically sound documentation.
  • Offer consultation on complex symptom management and goals-of-care discussions.

Interdisciplinary Collaboration & Leadership
  • Lead yourbi-weekly interdisciplinary team meetings, providing medical expertise for care planning and team decisions.
  • Work closely with Bloom primary care attending MDs and community providers to support seamless transitions, shared accountability, and continuity between primary care and hospice.
  • Collaborate with nursing, social work, spiritual care, and other team members; provide clinical guidance and education to enhance hospice/palliative best practices.

Quality, Compliance, and Clinical Excellence
  • Ensure alignment with CMS regulations, state rules, and Bloom policies through documentation, audits, and quality initiatives.
  • Contribute to symptom control, patient/family experience, and workflow improvements.

Qualifications
  • MD or DO with active Colorado license and DEA.
  • Board certification in Internal Medicine, Family Medicine, Geriatrics, Hospice & Palliative Medicine, or related field preferred.
  • Hospice, palliative, or serious illness experience strongly preferred.
  • Strong clinical judgment, communication, and interdisciplinary collaboration skills.

Work Environment & Expectations
  • Limited travel required - occasional visits to patient homes or facilities for required face-to-face encounters and select IDG meetings; most clinical work and meetings are remote/virtual.
  • Participate in shared remote call rotation (approx. every 6 weeks; team-based to promote sustainability).
