We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality, patient-centered care to individuals in the comfort of their homes. The ideal candidate is clinically strong, independent, organized, and passionate about helping patients maintain independence and improve their quality of life.
Key Responsibilities
  • Perform comprehensive patient assessments and develop individualized care plans
  • Administer medications, treatments, and wound care as prescribed
  • Monitor patient progress and update care plans as needed
  • Educate patients and family members on disease management, medications, and self-care techniques
  • Coordinate care with physicians, therapists, and interdisciplinary team members
  • Maintain accurate and timely documentation in compliance with agency and regulatory standards
  • Ensure compliance with state, federal, and agency regulations
Qualifications
  • Active Registered Nurse (RN) license in the state of practice
  • Graduate of an accredited nursing program (ASN or BSN preferred)
  • Minimum 1 year of clinical nursing experience (home health experience preferred)
