About the Job
An established Home Health/Hospice company is seeking caring RN's in the Houston Texas area and surrounding counties. We believe in delivering the best care to our patients and families and we are seeking staff who have the same beliefs. We are an Agency that care about our staff's health and their families we want to make sure they stay as healthy as possible by using increased safety protocols and increased patient screenings for infectious diseases. We partner with some of the worlds leading safety professionals in establishing safety protocols for our staff, their families, and our patients.
Job Details:
  • Currently licensed with the state of Texas.
  • Excellent verbal and written communication skills.
  • Experience with electronic medical records is strongly desired.
  • Home Health or hospice assessments including consents in the home, hospital or facility.
  • MINIMUM of 1 year of Hospice and Home health experience with verifiable references.
  • Wellsky/Kinnser experience REQUIRED.
  • Be available by phone during normal working hours, when on call, and when on back up call.
  • Performs patient visits according to the Plan of Care or as patient need dictates.
  • Deliver care and instruction that is appropriate to the patient's plan of care.
  • Coordination of care with wound care agencies, doctors and team members via Kinnser.
  • Perform case management activities for assigned patients.
  • Respond to verbal and written issues, concerns, or complaints from home care patients, families, physicians, nurse.
  • Comprehensive Assessments of patients utilizing the OASIS data sets or Hospice HIS data set.
  • May entail GIP care.
  • Meet with the IDT/case conference team once every other week.
  • Experience working with children highly considered.
  • Coordinate schedule to make most use of time during the day.
  • Will require driving outside of the immediate residential area to provide patient care.
  • Concierge Nursing required.
