We are seeking a compassionate and dedicated Registered Nurse (RN) to provide exceptional care to patients in their homes. This role involves coordinating patient care with the interdisciplinary team, families, and referring agencies, while ensuring the highest level of quality care.
Job Title: Registered Nurse (RN) – Adult Hospice and Home Health
Locations: Brownsville, San Benito, Harlingen
Job Types: Part-Time, PRN
Shifts: Day Shift, Evening Shift, Night Shift, and Weekends as needed
Work Schedule: Monday to Friday, Weekends, Evenings as needed
Key Responsibilities:
  • Patient Care: Deliver direct nursing care to patients in their homes, following established care plans and physicians' orders.
  • Assessment & Documentation: Monitor vital signs, document patient conditions, and maintain accurate medical records, including histories, diagnoses, and treatment plans.
  • Medication Management: Administer and monitor patients' medications, ensuring proper dosage and observing results.
  • Care Coordination: Collaborate with healthcare team members to assess, plan, implement, and evaluate patient care plans.
  • Patient Support: Provide personal care, medical attention, and emotional support to patients, ensuring their comfort and well-being.
  • Training & Supervision: Train and mentor other Registered Nurses, ensuring the effective coordination of care with the broader healthcare team.
  • Family Support: Communicate patient needs and provide support and guidance to families in the care process.
  • Other Duties: Perform additional duties as needed to ensure quality patient care.
