Our home health clinicians work with physicians to provide in-home rehabilitation to our patients. Together, we will help our patients achieve the outlined goals and milestones on their patient care plan.
Job Description:
As the RN case manager, you will plan, organize, and direct Hospice care for your patients. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. You will be the central point for the patient's care plan and will help lead and mentor our Home Health Aides.
Full time staff qualifies for benefits.
Responsibilities:
  • Performs comprehensive assessment for patient admission
  • Develops and updates the plan of care in consultation with the physician and all clinicians in charge of patient's care
  • Participates in the patient's discharge planning process.
  • Provides skilled nursing care as needed in accordance with the plan of care.
  • Cooperates with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan.
  • Always strives to improve nursing care by attending in-service education, through formal education, attendance at workshops, conferences, active participation in professional and related organizations.
  • Responsible for the overall clinical record maintenance to include visit note documentation be completed within 24 hours, comprehensive assessments, including OASIS, be completed within 48 hours, medication profile updated timely, orders/plans of care submitted timely, etc.
  • Maintains confidentiality of patients' private health information.

Qualifications:
  • Current Texas State license as a Registered Nurse
  • 2+ years' experience as a Registered Nurse in home health.
  • Reliable transportation
  • Current Texas Driver's License
  • Valid and current auto liability insurance
  • Must have Kinnser EMR experience
