What Youโ€™ll Do

As a Charge Nurse in a skilled nursing facility, you will provide high-quality care focused on preventing skin breakdown and promoting the healing of changes in skin integrity. You will assess and monitor residents, complete prescribed wound treatments, maintain accurate documentation, and collaborate with residents, families, physicians, and the interdisciplinary care team.

  • Provide quality wound and skin care to help prevent skin breakdown and promote healing in accordance with resident assessments and individualized care plansย ย ย ย 
  • Assess residents for changes in skin integrity and promptly communicate findings to the Director of Nursing or designeeย ย ย ย ย 
  • Perform prescribed wound treatments and ensure dressing changes are completed accurately and on timeย ย 
  • Monitor wound-healing progress and recommend changes to the plan of care when appropriateย ย ย ย 
  • Perform and document weekly pressure ulcer assessments and measurementsย ย 
  • Complete the Weekly Focused Skin Assessment, document audit results, and provide information to appropriate facility personnel during QA and care plan meetingsย ย 
  • Communicate skin-integrity concerns and resident-care updates to the responsible party and attending physician
  • Participate in training and in-service education related to wound prevention, skin care, and treatment practicesย ย 
  • Participate in the facilityโ€™s rotating nursing on-call scheduleย ย 
  • Assist with other nursing functions or duties as assigned by the Director of Nursing or Administrator

  • Must have an active RN license in the state of Texas in good standing
  • Must have graduated from an accredited RN nursing program
  • Previous skilled nursing, long-term care, or rehabilitation experience preferred
  • Wound Care Certification preferred
