Home Infusion Nurse - Partner With Patients. Practice With Purpose.

As a Home Infusion Nurse, you'll provide one-on-one, skilled clinical care to patients where they feel most comfortable-at home or in an ambulatory infusion suite. This role offers true autonomy, a flexible schedule, and the opportunity to build meaningful connections with patients and caregivers while delivering life-changing therapies.

You'll have the support of a multidisciplinary care team, comprehensive training, and the resources you need to deliver safe, effective, and compassionate infusion care.

In This Role, You Will:

  • Learn more about our Home Infusion Pharmacy: Optum Infusion Pharmacy
  • Work an average of 30-40 hours per week
  • Travel within the Lawrence, Topeka to Overland Park area to deliver care in patient homes or designated sites.

Key Responsibilities

  • Travel to patient homes and related sites using a personal or rental vehicle in accordance with company travel policies
  • Provide safe and compliant infusion services in accordance with Optum Infusion Pharmacy policies, protocols, and physician orders.
  • Establish and maintain positive therapeutic relationships with patients and caregivers, including education on self-care and self-administration.
  • Assess patient suitability for home infusion therapy, including home safety, caregiver support, and patient independence.
  • Obtain and maintain accurate patient medical histories, clinical assessments, and all required documentation, including plans of care, visit notes, medication profiles, and discharge summaries.
  • Deliver skilled nursing care across a range of patient ages and conditions based on physician orders and treatment plans.
  • Establish, access, and maintain vascular devices, including peripheral IVs, PICC lines, tunneled catheters, and implanted ports.
  • Administer medications and monitor patient responses, including managing infusion reactions, vital signs, side effects, and lab collection.
  • Communicate patient status and therapy response promptly with physicians, pharmacists, supervisors, and other care team members
  • Assist in staff orientation and training, serving as a clinical educator and resource as requested

Required Qualifications

  • Active and unrestricted Registered Nurse (RN) license in the state of Kansas
  • Current BLS/CPR certification from AHA or Red Cross or willingness to obtain prior to start
  • One or more years of RN experience in a clinical setting supporting infusion or IV therapy (e.g., ICU, Med-Surg, ED, PACU).
  • Ability to work independently with strong clinical judgment and effective interdisciplinary collaboration skills.
  • Valid driver's license, reliable transportation, and ability to cover an 80-mile service radius while working 30-40 hours per week

Preferred Qualifications

  • Previous home infusion experience
  • Familiarity with Microsoft Office and EMR platforms
  • Knowledge of Infusion Nursing Society (INS) standards
  • Understanding of HIPAA, OSHA, and home infusion accreditation guidelines
  • Strong communication, organization, critical-thinking, and teaching skills
