• Perform thorough psychosocial assessments to understand the complex needs of each client, ensuring a solid foundation for treatment planning.
• Diagnose and treat a variety of mental, emotional, and behavioral disorders, making adjustments to care plans as necessary to reflect changes in patient status.
• Coordinate with a diverse array of stakeholders, including family members, healthcare providers, community resources, and insurers, to facilitate an optimized care continuum.
• Actively engage in crisis intervention, providing immediate support and referring clients to appropriate services, such as support groups or treatment programs.
• Ensure compliance with federal, state, and local regulations as well as best practices in mental health care.
• Contribute to the development of discharge plans with a focus on continuity of care and patient well-being.
• Provide social work consultations and psychosocial interventions, including crisis bereavement support and other necessary therapeutic modalities.
• Advocate for patients, ensuring their needs and concerns are heard and addressed within the care team settings.
• Identify clients at risk for avoidable delays or complex discharge needs, collaborating with healthcare leaders to find effective solutions.

The clients you will serve come from a variety of backgrounds, facing numerous challenges related to mental health, including but not limited to anxiety, depression, trauma, and chronic stress. Your role is essential in providing both individual and family therapy, thus enhancing the overall family system's health and resilience. • A Master's Degree in Social Work or a closely related field from an accredited institution.
• Current state licensure as an LCSW, LMFT, LPC, or as a Psychologist.
• Familiarity with community mental health resources available in Georgia and a demonstrated ability to navigate those systems effectively.
• Excellent verbal and written communication skills, with a strong ability to foster rapport with clients.

• Experience working in a community health setting or similar environment.
• Knowledge of diverse therapeutic techniques and hands-on experience in their application.
• A record of successful collaboration within multidisciplinary teams.
• Prior experience with crisis intervention and management.

The position offers a competitive annual salary ranging from USD 90,000 to 100,000, commensurate with experience. Additional benefits include:
