  • Provide therapeutic support through individual and family sessions, effectively addressing crisis situations and facilitating referrals to community resources, support groups, or additional health services.
  • Engage actively with patients' caregivers by offering social work consultations and performing crisis interventions as needed.
  • Educate and coordinate with healthcare providers, insurers, and related agencies to facilitate a smooth transition for patients from clinical therapy to community integration.
  • Act as an advocate for your clients, ensuring their voices are heard in the care planning process, and identify patients at risk for extended stays or readmissions, coordinating solutions accordingly.
Patient Population / Client Focus

Your clients will encompass a diverse range of individuals experiencing various mental health conditions, including depression, anxiety, trauma, and more. The role necessitates a strong understanding of community mental health resources, as well as the ability to build rapport and tailor interventions to meet the unique cultural and social needs of each patient.

Qualifications

  • Proficient in using digital platforms for managing client care and documentation.
  • A strong ability to establish rapport and trust with clients, ensuring a safe therapeutic environment.
Preferred Experience
  • Experience working in a community health clinic or similar mental health setting.
  • Familiarity with teletherapy platforms and best practices in remote mental health counseling.
  • Previous collaboration with multidisciplinary teams to foster holistic approaches to patient care.
