Position Summary
The Intensive Case Manager provides comprehensive, individualized support to refugees and other eligible populations who have been in the United States for less than five years. This role focuses on assisting especially vulnerable individuals who face challenges navigating systems and achieving self-sufficiency.
You will work with diverse populations, including large families, women at risk, elderly refugees, LGBTQI individuals, and those with significant physical or mental health needs.
Key Responsibilities
  • Provide intensive case management services addressing needs related to housing, healthcare, education, and specialized client challenges
  • Develop individualized, holistic service plans based on client needs and goals
  • Conduct RHS-15 mental health screenings and coordinate appropriate referrals
  • Build and maintain relationships with community service providers and develop resource networks
  • Facilitate community support groups that promote connection and healthy coping strategies
  • Coordinate client appointments, transportation, and schedules in collaboration with internal teams and external partners
  • Provide health education, including Medicaid guidance and navigating healthcare systems
  • Refer clients to volunteer mentoring programs in coordination with the Volunteer Coordinator
  • Assist clients in resolving urgent or emergency situations
  • Participate in trainings and ensure compliance with all organizational policies and procedures
Qualifications
  • Bachelor's degree in social work, Psychology, Public Health, Community Services, or a related field preferred, or equivalent experience working with refugees or vulnerable populations
Preferred Experience
  • At least two years of experience in a culturally diverse environment or community services setting
  • Experience supporting refugees or similar populations
Additional Requirements
  • Valid driver's license with a clean driving record
  • Reliable personal transportation and proof of automobile insurance
  • Ability to travel locally to meet client needs
