Job description

We are seeking an experienced, transformational Chief Operating Officer to build and lead the operational foundation of our rapidly growing Medicaid Home Health, PDN, PAS, and Hospice organization. This is not a traditional COO role, we need an executive who can build the systems, leadership structure, and performance culture required to scale the company from approximately $15 million to more than $150 million in annual revenue over the next 3 to 5 years.

The COO will own all operational functions i.e. clinical operations, staffing, scheduling, intake, admissions, quality, compliance, workforce productivity, and customer experience, working closely with the CEO, CFO, and Chief Growth Officer to deliver exceptional patient care while building a scalable platform for future expansion and acquisitions.

Key Responsibilities

• Lead day-to-day operations across PDN, PAS, and Hospice; translate strategic vision into operational execution

• Build and develop a high-performing leadership team

• Design and continuously improve scalable systems for intake/admissions, clinical operations, scheduling, care coordination, QAPI, compliance, and KPI reporting

• Develop standardized operating procedures supporting consistency, acquisitions, and multi-location growth

• Improve patient, caregiver, and referral partner experience; increase scheduling efficiency and caregiver utilization

• Recruit, develop, and mentor operational leaders; partner with HR on recruiting, onboarding, retention, and engagement

• Build operational dashboards tracking productivity, quality, staffing, satisfaction, retention, and admissions



RequirementsRequired

• Bachelor's degree in Healthcare Administration, Nursing, Business, or related field (Master's preferred)

• 10+ years of progressive healthcare leadership experience, including executive operational leadership

• Demonstrated success building scalable operational systems and leading organizational transformation

• Strong leadership, communication, and change management skills

Preferred

• Experience in Home Health, Hospice, PDN, or PAS

• Knowledge of Medicaid operations and regulatory compliance

• Experience leading multi-site healthcare organizations

• Experience supporting acquisitions, expansion, or rapid organizational growth
