Job description

Pediatric Home Service is a friendly and professional leader in home health care with a singular passion for and commitment to taking care of the child . As a leader in home health care, we help kids achieve their best lives at home and in their communities while providing our employees a fun and rewarding place to work.
  • Position: Respiratory Therapist
  • Location:Madison/Milwaukee, WI
  • Hours: Monday-Friday, 8am-5pm. Weekends/On-call: as determined by leadership
  • Requirements: holds the following license; LRT, BLS

Sign On Bonus $3,000
THE POSITION:
  • Uses analytical ability to perform initial patient assessments including physical, functional, social and environmental resource requirements and needs. Ensures communication and coordination of services /care among all clinical services.
  • Performs initial equipment set-up and patient/family/caregiver education.
  • Develops patient specific plans of care based on initial assessments to include problems, goals and interventions.
  • Reviews and revises care plans on an ongoing basis and ensure that all patients are followed according to that plan per PHS policy.
  • Ensures that all required documentation is completed accurately and within the time frames set forth in PHS policy.
  • Participates in care conferences and collaborates with all care team members internally and externally.
  • Participates in day to day trouble shooting. Performs all on-call activities, analyze problems, formulates solutions, and notifies manager on-call as needed.
  • Attend department and staff meetings as necessary.
  • Schedule all patient visits per PHS follow-up policy.

THE QUALIFICATIONS:
  • Certified Respiratory Therapist (CRT) by The National Board of Respiratory Care (NBRC)
  • Current Basic Life Support (BLS) certification required
  • One to two years of pediatric experience as a Respiratory Therapist

Preferred Qualifications:
  • Neonatal & Pediatric Specialist (NPS)
