About the Role
We are looking for a compassionate Nurse Practitioner to join our hospice team.
This role is ideal for someone dedicated to providing comfort, dignity, and high-quality palliative care to patients and their families during life's most meaningful moments. If you are collaborative, patient-centered, and passionate about hospice care, we would love to connect with you.
Current positions available:
  • PRN
  • Part-Time
  • Full-Time

Responsibilities
  • Conduct required face-to-face hospice visits and report findings to the Hospice Medical Director
  • Develop, coordinate, and document patient plans of care
  • Collaborate with attending physicians and IDG members
  • Prescribe medications (including controlled substances) within scope and delegation
  • Order diagnostics and treatments as appropriate
  • Educate patients, families, staff, and community on hospice and palliative care
  • Participate in IDG and QAPI meetings
  • Maintain accurate clinical documentation for compliance and reimbursement
  • Supervise and evaluate home health aides as appropriate
  • Complete required training modules
