About the Role
We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide high-quality, compassionate in-home nursing care. This role supports patients' health, safety, and daily care needs as part of an interdisciplinary team.
Responsibilities
  • Provide skilled nursing care per physician orders and care plans
  • Monitor patient condition, assess pain, and educate patients and families
  • Administer medications safely following the 7 rights
  • Ensure patient safety and maintain confidentiality
  • Communicate patient status and needs with the care team
  • Complete accurate and timely documentation
  • Participate in required trainings, in-services, and meetings
  • Follow agency policies, procedures, and emergency protocols

  • Active LPN license in the state of Indiana
  • Recent acute care or home health experience (home health preferred)
  • Valid driver's license, reliable vehicle, and auto insurance
  • Current TB test and CPR certification
