Job description

Float Physical Therapist (PT) in Home Health

Schedule: Full-time | 80 Hours/Pay Period | Monday to Friday – 8 a.m. to 4:30 p.m.

Pay range: $41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.

Home Branch: Alexandria, MN OR Baxter, MN. Position will float across the Vivie continuum.

As a Float Physical Therapist (PT) in Home Health, you'll play a key role in providing flexible, high-quality clinical coverage for home health clients throughout Vivie's footprint. This position supports patient care during staff absences or open positions, delivering skilled physical therapy and case management as needed. Through clinical expertise, collaboration, and adaptability, Float PT ensures that every client receives the care and attention they deserve, no matter where they live.

As a Float Physical Therapist, you will:

Provide Direct Client Care

  • Evaluate clients by obtaining and interpreting subjective and objective data. Develop personalized treatment plans with short-term and long-term goals in accordance with physician orders. Deliver high-quality care that empowers clients to achieve their functional goals.

Maintain Accurate Client Records and Billing

  • Document all services accurately and promptly using Vivie's EMR system. Demonstrate a thorough understanding of all payer sources and authorization requirements. Complete and document supervisory visits with PTAs and/or HHAs in compliance with regulatory guidelines.

Case Management & Continuity of Care

  • Serve as primary client coverage for staff members on FMLA, sick leave, vacation, or during open positions. Manage cases as needed based on duration of coverage, including all visit types and coordination with the interdisciplinary team.

Communicate Effectively and Collaboratively

  • Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team. Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery.

  • Current Physical Therapist license in the state of Minnesota.
  • CPR certification required within 6 months of hire.
  • Willingness to travel across the region as necessary.
  • Valid driver's license and access to an insured personal vehicle.
  • Ability to pass state mandated background check.
  • Ability to read, write, and speak English proficiently to communicate effectively with clients, families, and team members.
  • Physical capability to perform job duties.
