Description

The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y. 


Jordan Health is now seeking a MAAT Clinic RN who, under the guidance of the Chief of MAAT Clinic, is primarily responsible for pre-treatment screenings, intake evaluations and induction of patients under close supervision of Chief and working with the MAAT Practice Manager and MAAT team. The MAAT Clinic RN will assist MAAT Practice Manager with administration and management of the MAAT clinic.

Requirements

The MAAT Clinic RN will ensure to:

Assist with medical screenings of patients being considered for MAAT services with special attention to cardiopulmonary disease like HIV, Hep C, Hep B and Tuberculosis screening. 

Obtain prior authorizations for MAAT related medications and facilitate the coordination of MAAT related medications in terms of following up on prescriptions, prior authorizations, ensuring safe, appropriate and timely delivery and receipt of medication, storage and administration. 

Assist the MAAT coordinators

Assist with program planning and activities in accordance with funding requirements and policies and procedures of Jordan Health center. 

Assist with providing direct client intervention/counseling as necessary to MAAT clinic patients. 

Assist with planning and presenting in service or professional development training to MAAT clinic staff. 

Other duties as assigned


Education And Experience Required:

Bachelor's or Associates degree in nursing with Master's preferred. 

Minimum of 1 year experience with Addiction Medicine experience preferred


Licenses And Certifications: 

Current NYS RN License without any negative actions 

Current provider BLS/CPR through the American Heart Association. 

Infection Control Certification

Certification in addiction nursing preferred


Special Skills, Knowledge Required:

Experience with Microsoft Office including Word and Excel is required. 

Fluency in Spanish preferred but not required. 

Having a valid driver's license and a vehicle is strongly preferred as position will require travel to 3 comprehensive sites at minimum.
