About the Role:
Star Light Home Health is looking for a compassionate and dedicated LPN Home Health Visit Nurse to join our growing team in Montrose. We service from Grand Junction, CO. to Ouray.   This is a rewarding opportunity to make a meaningful difference in patients' lives by delivering high-quality, personalized care right in the comfort of their homes. If you're a skilled LPN who thrives in an independent yet supportive environment, we want to hear from you!
Responsibilities:
  • Conduct skilled nursing visits to patients in their homes throughout the Grand Junction area
  • Perform assessments, wound care, medication administration, and other clinical nursing duties
  • Monitor and document patient health status, progress, and changes in condition
  • Communicate effectively with RNs, physicians, and care team members to coordinate patient care plans
  • Educate patients and family members on care techniques, medications, and health management
  • Maintain accurate and timely clinical documentation in compliance with home health regulations
  • Provide compassionate, patient-centered care while promoting independence and dignity
Requirements:
  • Active LPN license in the state of Colorado (required)
  • Prior home health, visiting nurse, or field nursing experience preferred
  • Strong clinical assessment and wound care skills
  • Reliable transportation, valid driver's license, and auto insurance
  • Proficiency with electronic health records (EHR) and clinical documentation
  • Excellent communication, time management, and organizational skills
  • Compassionate, self-motivated, and able to work independently in a field-based setting
