Job description

First Choice Health Centers (FCHC) is a Federally Qualified Health Center (FQHC) with outpatient nursing opportunities in East Hartford, CT!  Our Staff LPNs are primarily responsible for managing urgent and emergent patient calls across all disciplines, in addition to performing licensed professional nursing functions to assess the needs and severity of patients requesting healthcare services.  The LPN functions as an essential member of the healthcare team, facilitating patient medication refills, managing providers' schedules, counseling patients and other related tasks as needed.

Minimum Knowledge, Skills & Abilities Required:

    Experience with an electronic health record systemVaccine Administration experienceExcellent communication and interpersonal skillsA positive attitude and willingness to be cross-trained in multiple specialty areas, including pediatrics
Experience and Training:
    A valid LPN license in the state of ConnecticutBLS Certification
Standard Job Duties:
    Provide direct and individualized nursing care to patients based on the application of the nursing processMaintains accurate medication inventory listings, oversees the CT Vaccine Program in accordance with Center policies and requirements including monthly inventory, ordering and documentation on each patient in the electronic health record with required elementsAccurately documents in the electronic health record, all assessments, vital signs, specimen collections, interventions, evaluations and responses, teaching, procedures, narratives, referrals, etc. in the proper patient recordManages prescriptions and medication refills for providers in accordance with the Nurse Performs wound care using appropriate infection control practicesPerform periodic meaningful use documentation including depression screening, sexual history, alcohol and substance abuse history and assessment. Refers appropriate patients to specialty providers in relation to the responses to the noted assessmentsWorks with medical assistants to ensure safe and effective patient care is rendered in accordance with the medical assistant's scope of practiceInteracts with outside agencies to manage patient care outside the CenterMonitor task list (telephone encounters) in order to respond in a timely manner, follow up on overdue orders and monitor status of critical referralsAdhere to all patient safety standards and infection control guidelinesAssist care team to assure smooth office operation and delivery of excellent service through teamwork.
