Job Summary


The Program Manager reports to the Director of Strategic Programs and is responsible for overseeing the day-to-day operations of assigned programs and grant-funded initiatives. This position provides leadership in stakeholder engagement, program implementation, grant compliance, data management, reporting, quality improvement, and cross-sector collaboration. The Program Manager works closely with internal staff, Tribal Nations, healthcare providers, behavioral health organizations, community partners, and funding agencies to ensure successful program delivery, achievement of organizational goals, and compliance with contractual and regulatory requirements while advancing Axiom Care's mission throughout Cibola, McKinley, San Juan, and Sandoval Counties in New Mexico.



Duties/Responsibilities

Develop and maintain organized databases and tracking systems for stakeholder contacts, partnership activities, referrals, meetings, outreach efforts, implementation milestones, grant deliverables, and program performance.

Maintain accurate, secure, and confidential program records in accordance with agency policies, HIPAA requirements, funding regulations, and applicable state and federal guidelines.

Monitor program data and performance metrics, identify trends, service gaps, and areas requiring attention, and provide recommendations to improve program effectiveness and implementation outcomes.

Prepare weekly, monthly, quarterly, and annual reports for program leadership, executive leadership, funding agencies, and community partners, as required.

Coordinate the collection, review, and timely submission of grant documentation, performance measures, deliverables, and required program reports.

Monitor program activities to ensure compliance with grant requirements, contractual obligations, organizational policies, accreditation standards, and reporting deadlines.

Assist program leadership in coordinating and monitoring implementation activities of Community Engagement Coordinators, Tribal Partnership staff, and other assigned personnel to ensure alignment with organizational priorities, grant objectives, and approved work plans.

Support continuous quality improvement (CQI) initiatives by monitoring performance indicators, documenting progress, identifying operational challenges, and recommending process improvements that enhance service delivery and program outcomes.

Assist with gathering documentation and preparing information necessary for reimbursement requests, budget monitoring, grant reporting, audits, and other administrative or financial requirements.

Develop and maintain a comprehensive understanding of Axiom Care's programs, services, organizational policies, grant requirements, and applicable local, state, Tribal, and federal regulations.

Participate in professional development opportunities, trainings, conferences, learning collaboratives, and technical assistance activities to strengthen program operations and leadership competencies.

Maintain the highest standards of professionalism, confidentiality, ethical conduct, cultural humility, and trauma-informed practices in all interactions with clients, staff, partners, and community stakeholders.

Participate in multidisciplinary team meetings, strategic planning sessions, community coalitions, organizational initiatives, and cross-functional workgroups while promoting collaboration, effective communication, and shared decision-making.

Assist with the development, implementation, and evaluation of new programs, community initiatives, and grant-funded projects that advance Axiom Care's mission and strategic priorities.

Perform other duties as assigned to support the successful implementation and sustainability of Axiom Care's programs, services, and community initiatives.

Professional and/or personal experience with substance abuse treatment highly preferredย 


Physical Requirements:

Prolonged periods of sitting at a desk and working on a computer.

Must be able to lift up to 15 pounds at times.
