Position Description
Are you passionate about helping others on their journey to wellness and recovery? We're looking for a Recovery Plan Coordinator (RPC) to join our team. In this role, you'll coordinate, organize, and monitor treatment and rehabilitative services to support individuals in achieving their goals. Working under the guidance of a team lead, you'll ensure recovery plans are thoughtfully designed, maintained, and adapted to meet the unique needs of those we serve.
What You'll Do
  • Lead the recovery planning process by developing, reviewing, and updating individualized Recovery Plans.
  • Organize and facilitate treatment team meetings, ensuring input from case managers and other providers.
  • Track progress and participation in services, making adjustments as needed.
  • Advocate for individuals served, ensuring personalized, strengths-based supports are in place.
  • Refer individuals to appropriate rehabilitation services based on needs.
  • Collaborate across disciplines to strengthen overall service delivery.
  • Provide guidance and training to team members on psychiatric rehabilitation principles.
  • Deliver services with cultural and age-appropriate competency.

  • Experience: Previous experience completing treatment plans (required).
