We're looking for a compassionate and motivated Registered Nurse Case Manager to join our team, supporting Northern Virginia. This is a field-based role that blends in-person visits with telephonic outreach to deliver coordinated, patient-centered care.

Schedule: Mondayโ€“Friday, 8:00 AM โ€“ 5:00 PM EST

What You'll Do:

As a Nurse Case Manager, you will play a critical role in improving the health and well-being of members by coordinating care, removing barriers, and supporting better outcomes.

In this role, you will:

  • Conduct comprehensive assessments (telephonic and face-to-face) to evaluate members' medical, behavioral, and social needs
  • Develop and implement personalized care plans that promote both short- and long-term wellness
  • Coordinate care across providers, programs, and services to ensure a seamless experience for members
  • Use clinical expertise and data insights to identify risks, gaps in care, and opportunities for intervention
  • Support members with complex or multiple conditions by applying a holistic, whole-person approach
  • Facilitate referrals to appropriate clinical and community resources
  • Participate in case conferences and collaborate with multidisciplinary teams to optimize care strategies
  • Review claims history and benefit eligibility to inform care planning
  • Engage members using strong interviewing skills to build trust and encourage active participation in their care
  • Ensure all case management activities comply with regulatory and organizational guidelines
Required Qualifications:
  • Active, unrestricted Registered Nurse (RN) license in Virginia
  • Residence in Northern Virginia in one of the above mentioned locations
  • Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy
  • 3+ years of clinical nursing experience (e.g., medical-surgical, acute care, or behavioral health)
  • Strong communication, organization, and critical-thinking skills
  • Comfort working with technology, including multiple systems and documentation platforms
Preferred Qualifications:
  • Experience in case management, care coordination and/or managed care organizations.
  • Discharge Planning and/or transition of care experience
  • Bilingual (English/Spanish)
  • Ability to thrive in a fast-paced, dynamic environment and manage multiple priorities effectively
Education:
  • Associate Degree in Nursing (required)
  • Bachelor of Science in Nursing (BSN) (preferred)

Anticipated Weekly Hours: 40

Time Type: Full time

Pay Range: The typical pay range for this role is:

$60,522.00 - $129,615.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
