RN Case Manager - Per Diem in St Cloud

Join our compassionate team at BrightStar Care of Maple Grove & Andover and take your career to the next level! We're on the hunt for an amazing Part Time RN Field Case Manager!

Pay Rate: $42–$45 per hour

Work Schedule: Per Diem

Traveling to a client's home within our territory is a requirement (St. Cloud) We are just expanding to the St. Cloud Community.

Responsibilities of an RN Case Manager:

  • No OASIS work!
  • Provides care as defined within the scope of the state's nurse practice act.
  • Skilled Cares include, but not limited to: Wound Cares, INR, Catheter Cares, Medication Management, and more.
  • Makes the initial evaluation visit and regularly reevaluates the client's nursing needs.
  • Prepares clinical and progress notes and summaries of care.
  • Provided On-Call Coverage in rotation with other RNs on team (Additional $200/week pay)
  • Assures proper maintenance of clinical records in compliance with local, state, and federal laws.
  • Helps educate and coach care-giving staff and assist with new hire clinical training, as needed.

RN Case Manager Requirements:

  • Current unrestricted RN license in Minnesota
  • Minimum two (2) years of clinical experience with one (1) year of experience in a home care agency, primary care clinic, or health facility, preferred.
  • Active CPR Certification
  • Negative TB skin test or QuantiFERON-TB Gold (QFT) blood test (completed within the last 90 days)
  • Reliable car & Auto Insurance
  • Valid Driver's License
  • Clean background and Criminal Record
  • Understands regulatory compliance with state and federal regulations.
  • Ability to work on call rotation with compensation
