We are looking to add an Advanced Practice Provider to join our Family Practice Team! The Advanced Practitioner plays a key role in providing the highest quality of patient care. Our providers deliver exceptional patient care with a focus on overall wellbeing of our patients.


Nurse Practitioner/PAjob details:

Essential Job Duties:

  • Completes the routine physical assessment using common assessment skills in a systematic manner.
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
  • Records physical findings, formulates plan and prognosis, based on patient's condition.
  • Performs physical examinations and preventive health measures within prescribed guidelines and instructions of physician.
  • Respects patients' rights, including the right to inform, privacy, confidentiality, and the right to refuse care.
  • Assists with patient education to promote self-care and patient independence.
  • Follows company policies and procedures and department protocols.

Schedule:

  • Full time (M- F), 40 hours per week (8am 5pm)

Qualifications:

  • Graduate from an accredited MSN or PA program
  • Must have Indiana RN license.
  • Must have active APRN or PA/CSR license or be eligible for licenses.
  • AANP, ANCC or PA certification
  • Valid driver's license
  • Must have BLS certification
