Job Description and Guidelines



Tuesday- Saturday (off on Sunday and Monday) 


General: This position is responsible for providing direct care to residents of an 12  licensed specialized adult foster care home.  Management responsibilities primarily consist of providing direction and supervision to employees, being on-call every third week, and assisting with management duties as needed.  Typical hours are 3pm to 11pm, but may include after hours communication, filling in shifts, or accompanying residents to the hospital.


Requirements:


  • Be able to perform fine motor skills with fingertips (ex. van lifts and tie downs,) as well as be ambulatory and have sufficient agility to bend, rotate head, neck and torso, kneel, lifting and transferring people in wheelchairs and navigate stairs in order to perform all duties essential to the health, safety and well-being of the people we support.

  • Be able to read as well as communicate/speak and write accurate, legible, comprehensive written documentation in order to complete required training, provide cognitive skill training to the people we employ, complete compliance paperwork, participate in program implementation, and to complete all required documentation. Able to communicate/speak expressively and receptively for all aspects of job responsibilities. Have legible signature on all documentation. Any falsification of documentation is strictly prohibited and must be reported immediately to the Corporate Compliance Officer.

  • Be able to have a positive attitude and friendly manner, create a positive, caring culture of gentleness for the people we employ and the people we support. Managers must be able to communicate appropriately with employees, regulatory officers, agency workers, and the people we support. Be able to adhere to and role model the CCH Code of Conduct, and have knowledge of and adhere to the policies and procedures of CCH contracting agencies, Licensing Agencies, the State of Michigan and any other pertinent regulatory agencies when on duty.

  • Be able to act as an advocate for the people we support at all times in order to encourage and calm people supported as needed. Be able to; ensure recipient rights, safety, dignity, and privacy requirements have been met. Be able to intervene if you witness a person-supported rights are being violated; then immediately report.

  • Be able to provide services as they are written in the Person-Centered Plan, Behavior Plan, and Assessment Plan by training/monitoring/assisting the people with the level of support needed to implement their Plan of Service.

  • Be able to have good judgment, including the ability to evaluate, make decisions and anticipate consequences. Have interpersonal sensitivity (objective awareness of others, tactful interaction). Have insight and the ability to work autonomously.

  • Have a good understanding of the individualized needs of each person supported through their Personal Profile, all Assessments and ALIS information, which includes information on their medical, social, developmental and support needs. Be able to interact and engage in conversation and activities with the people supported. This includes talking/explaining what you are doing while working with people who may have limited verbal communication skills.

  • Be able to maintain accurate expense records and receipts for all petty cash, personal funds, and other purchases made on behalf of the people we serve or Cretsinger Care Homes, Ltd. Misuse of CCH’s and/or people’s funds will be prosecuted by law. Employees are never to borrow money from a person supported and may not benefit in any way from a person supported personal funds, belongings, or families.

  • Be able to be organized and demonstrate efficient time management.

Responsibilities regarding residents:


  • You are “on the floor” and all aspects of job performance regarding resident care are expected to be carried out accurately, professionally, and compassionately

  • Provide personal care with the appropriate level of physical assistance or verbal prompting: toileting, bathing, grooming, dressing, transferring, oral care, etc.

  • Have familiarity with each individual’s person centered plan and (if applicable) behavior treatment plan so you can be constantly aware of proper implementation, special considerations, and needed changes or updates to any existing plans

  • Dispense medications in accordance with CCH medication administration protocols

  • Prepare and serve meals in accordance with posted menu, following dietary guidelines

  • Wash laundry

  • Clean – dishes, sweeping, mopping, vacuuming, folding laundry, cleaning bathrooms, picking up clutter, washing tables, under beds, washing walls, picking up cigarette butts/trash outside, taking trash to dumpster, etc.

  • Complete all shift documentation


Responsibilities regarding employees:


  • Role model ethical and interactive behavior

  • Role model adherence to all policies, laws, and guiding principles, paying special attention to Personnel Policy Code of Ethics

  • Provide regular supervision and positive, constructive feedback to employees regarding their job performance, resident interaction, self-motivation, and attitude.

  • Provide immediate and effective guidance to correct poor job performance or policy violation, follow up with disciplinary action as warranted

  • Listen, coach, and guide employees in all circumstances while remaining professional and holding them to the employment standard

  • Follow the established communication system with employees to keep them apprised of any necessary information regarding residents, employees, policy, business, or facility updates.

  • Address policy violations, complaints, or concerns immediately

  • When disciplinary action is warranted, carry it out immediately and document appropriately – home manager will add written documentation to BambooHR. Conduct conversations in private

  • Keep employee information confidential, sharing only with home manager

  • Assure all required daily responsibilities are being completed – outings, activities, therapy, showers, meal prep, documentation, cleaning, etc.

  • Oversee meals – assure menu is being followed, food is properly/timely cooked and served, food is prepped according to resident (puree, thickened, etc.)

  • Confer with management team for information regarding your job duties such as employee oversight, medication administration, documentation, and inventory checks

  • Training – “hands on” training of new employees, as needed

    • Train new employees in accordance with current CCH guidelines and curricula

    • Accurately complete documentation for training


Other Management Responsibilities:


  • Be the on-duty manager while you are on the clock

  • Be on call every third week to answer questions, direct, and problem solve. Return emergency calls within 30 minutes, non-emergency within 60 minutes.

  • Be on call to fill shifts and accompany residents to the hospital every third week or as needed

  • Attend all staff meetings, training sessions, and other meetings as required

  • Assure supplies are available on your shift – cleaning, toilet paper, paper towels, etc.

  • Oversee medication administration on your shift; check that the med cart is organized and kept locked; inventory as needed

  • Assure staff documentation is completed on your shift

  • Communicate with home manager regarding any issues on your shift

Hiring Criteria:

  • Prefer experience managing employees
  • Two written, verified letters of reference speaking to character and job related skills
  • Valid driver's license
  • Clean criminal history - we are required to run background checks for all employees, including fingerprints, SAM, OIG, Recipient Rights, and more
